I understand and agree that services provided by Nurse Tanya Candee, RN, and Awakened Wellness LLC are for wellness education and coaching only. They do not diagnose, treat, or cure medical conditions, bill insurance, or provide medical advice. Functional lab testing and all information shared are for educational purposes only and are not a substitute for care from my physician or other licensed healthcare provider.
I take full responsibility for my health decisions and will consult my physician before starting or changing any medication, supplement, diet, or treatment. No outcome is guaranteed.
All services and supplements purchased through the Taste of Transformation program & The Great Health Reset are non-refundable. Program subscriptions may be canceled at any time, but no refunds will be issued for past charges.
Participation creates a client–coach relationship only and does not replace medical care.
